TRANSITIONAL AGE YOUTH (AGES 16-25)


This program serves transitional age youth (TAY), ages 16-25, who are diagnosed with a serious mental illness, who may have a substance use disorder and who are at risk of homelessness, incarceration or hospitalization. The program provides supportive, social rehabilitation services in a residential program. There is also the ability to live independently throughout the county.

The program supports young adults in moving toward personal recovery by providing stabilization and skill development to live an independent and successful life within the community. Services include group therapy, medication management and rehabilitation services. We also help young adults with their housing needs.
